What personal data is collected and why?
At the Initial Consultation a form is completed with a New Client Record these forms are to obtain information and are kept securely.
An Agreement is read through by the client and signed with agreement.
The Agreement contains at least a 24 hour notice of cancellation.
The therapist Heather Fletcher may discuss the client’s case with a qualified supervisor from the National Hypnotherapy Society or the National Counselling Society. The client understands that this is a standard practice and enables the therapist to provide the best possible treatment. There is an understanding that both the therapist and the supervisor are bound by the codes of ethics and confidentiality of the above organisations.
If there is a situation where the client poses a risk of causing harm to themselves or to others, then the therapist reserves the right to break the confidentiality agreement and seek appropriate advice.
In the case of a criminal act being committed, then the therapist is bound by law to break the confidentiality agreement and seek appropriate advice.
The client signs in agreement to the terms and that the information to the best of their knowledge is true and complete.
A signature is obtained from a parent if the client is under the age of 18.
